Jacob Henry Jack Stevens, 16, of Mt. Perry, passed away on Thursday, June 6, 2013 at his home due to an accidental gunshot. He was born on July 23, 1996 at O.S.U. Medical Center to Matthew and Jody (Harter) Stevens. Jack attended Sheridan High School and just completed his sophomore year. He was active in Sheridans wrestling and football teams, the Fellowship of Christian Athletes, and the Sheridan FFA. He also was a member of the Glenford Harvester 4-H Club, the 4-H Jr. Leaders of Perry County, the Jr. Fair Board of Perry County, and a member of the Lighthouse Memorial Church, Millersport where he sang in the choir.Jackis survived by his loving family: parents, Matt and Jody; four brothers and two sisters: Joshua Murphy, Sean (Tawnya Thorp) Hupp, James (Amy) Hupp, John (Kari) Stevens, Stephanie (Ben Amspaugh) Hupp, and Emily Stevens; paternal grandma, Janice (Ray) Dean; maternal grandma, Judith (Bill) Wilson; step-grandma, Evelyn Stevens; nieces and nephews: Aden, Izayah, Iyannah, Madison, Logan, Wyatt, and Ellie; many aunts, uncles, cousins, and friends, especially Ryan Ardrey; Ed (Shantel) Bennett and son, Eddie; friends of the wrestling team and Coach Chris King; FCA teacher, Paul Culver III; VoAg teachers, Mike Skinner and Rob Brokaw; and Advisory teacher, Greg Booth; as well as many other teachers and advisors who made an impact on Jack; and his beloved dog, Radar.Jack was preceded in death by his paternal grandpa, Donald Stevens; maternal grandpa, Fredrick Harter; uncle, Jeff Harter; as well as other aunts and uncles.A funeral service, celebrating Jacks life, will be held on Tuesday, June 11, 2013 at 7:00 p.m. in the Lighthouse Memorial Church, 2600 Walnut Rd (St. Rt. 79) Millersport, OH 43046, with Pastor Bill Beard officiating.The Family will receive friends at the Lighthouse Memorial Church, on Tuesday from 2:00 p.m. until the time of services at 7:00 p.m.In Jacks honor, please do not send flowers, but rather make memorials to: The Wounded Warrior Project, P.O.
